The Tdap and Meningococcal Vaccine is required for ALL Students Entering 7th and 12th Grade. Families can address this requirement through the school clinic in partnership with Valley Health Care, as they are offering the vaccine at schools now.
The following two forms can be downloaded and are needed for permission Immunization Consent Form and Contraindication Form. You can also contact your school nurse to have these forms sent home with your child. If you would like your child to receive the vaccine at school, please review, sign, and return the form as soon as possible.
Valley Health Care is available according to their schedule days at EHS, EMS, and TVMHS. This is a convenient way for your child to receive their required immunization without missing school.
